Short Report: Role of Electroencephalography in the Management of Aneurysmal Subarachnoid Hemorrhage: National Inpatient Sample, 2012-2017

This short report presents the findings of a study to investigate the association between EEG and outcomes and resource utilization after aneurysmal subarachnoid hemorrhage (aSAH) using the National Inpatient Sample from 2012 to 2017 (n=41896). 3.8% received EEG monitoring which was associated with higher cost and longer length of stay. These patients had a higher severity aSAH and EEG use significantly lowered risk of in-hospital death.

Review: Interpreting Intraoperative EEG; Beyond Indices to the Essential Role of Raw EEG and Density Spectral Array

This review article discusses common sources of discordance between the numerical index generated by processed EEG and raw EEG or density spectral array (DSA). The authors illustrate this through clinical examples to demonstrate the importance of raw EEG and DSA in routine clinical practice.

Review: Effects of volatile versus intravenous anesthesia on cerebral vasospasm in open surgical and endovascular procedures for aneurysmal subarachnoid hemorrhage: a systematic review and meta-analysis

This systematic review and meta-analysis investigates the effect of volatile anesthesia compared with total intravenous anesthesia on perioperative outcome in patients undergoing surgery for subarachnoid hemorrhage. Nine studies comprising 1459 participants were included. The authors were not able to demonstrate a difference in outcome when both cohort and randomized controlled trials (RCT) were included in the analysis and highlight the need for further RCTs.

This focused review examines the current evidence for timing of surgery after ischemic stroke including impact on perioperative outcomes, risk of recurrent stroke and areas where further evidence is required.

Clinical Investigation: Emergency Conversion to General Anesthesia During Endovascular Therapy and Stroke Outcomes: A Retrospective Matched Study

The aim of this retrospective cohort study was to investigate the impact of emergency conversion to general anesthesia in patients undergoing endovascular thrombectomy. The primary outcome was the modified Rankin score (MRS) at 90 days. The authors found that GA-conversion may adversely affect neurological outcome. Independent predictors of GA conversion were absence of atrial fibrillation and high National Institutes of Health Stroke Scale scores.
